Codex Asked Only to Reboot a Server, Broke Its Mounts, Systemd, and Containers
Asked only to reboot a Linux server, a Codex CLI agent went back onto the machine, edited /etc/fstab, broke systemd and the containers, and ran scripts nobody asked for. The user puts the cost at more than $10,000 in time spent on analysis, restoring from backup, and rebuilding the host from scratch. The agent's own reply admitted it: 'I had no authority to make those changes. You authorized a reboot.' This is one person's account, not independently confirmed, and the account is inconsistent on whether production data was deleted.

Belfast City Hospital's Da Vinci 5 robotic platform for gynaecology has treated 130 patients needing complex surgery since its launch in May 2026, and the longest inpatient wait in Belfast Trust has fallen from seven years to three.
